How to apply: when you click "Apply Online," you will arrive at a form, which will ask for your contact information and for information about how you use the internet now, and what you already know. Any question on the form that has a
red asterisk (*) is required. This means that you can't complete the form without responding to that question.
If you prefer, click this link to a printable PDF to download a Portable Document Format file, which can be printed on legal sized paper. You can fill that out by pen and drop it off or mail it (instructions are on the form).
Or, point your phone camera, or the camera on your tablet, at the QR code below. That's another way to get to the online form.
The square image on the right is a QR code (QR means "Quick Response"). It's a shortcut to our survey / application form.
